For Patients & Families
Care that meets you at home, in your community, and on your terms.
Whether you're starting a pregnancy, recovering from a hospital stay, or looking for residential support — here's what to expect when you work with us.
What to expect
A simple, respectful experience.
We listen first
Every member meets a real person — not a phone tree — to understand what they need.
We come to you
Most services happen in your home, your community, or virtually.
We coordinate
We talk to your doctors, MCO, and family so you don't have to repeat yourself.
How to get services
Three ways to start.
Ask your MCO or doctor
Have your care manager or provider send a referral to the right agency.
Call or email us
Reach our intake team directly and we'll guide you through eligibility.
Use our online form
Complete our short contact form and we'll respond within one business day.
Insurance & Medicaid
We accept Maryland Medicaid and most HealthChoice MCOs.
If you're unsure whether your plan is covered, call us — we'll verify benefits and help you understand what services are available at no out-of-pocket cost.
